KEVIN DORSETT, M.D.
BOARD CERTIFIED OPHTHALMOLOGIST
Dr. Dorsett performs the latest cataract surgeries, including Femtosecond® Laser Cataract surgery, the greatest advancement in cataract surgery in over 40 years and also offers specialty IOL options for his patients offering improved vision across the spectrum from far to near. Many patients will find they no longer need to wear glasses after their cataract surgery.
A graduate of the University of South Florida School of Medicine where he did his ophthalmology residency, he entered practice with Lakeland Eye Clinic over fifteen years ago. Dr. Dorsett supports many local community activities to include seeing patients for Lakeland Volunteers in Medicine. He is willing to speak to local groups regarding both Eye Care for Seniors and Advances in Eye Care.
Having grown up in Florida, Dr. Dorsett enjoys sailing, fishing and time spent both on the water and camping in our area’s beautiful Everglades. He especially enjoys spending time with his two daughters and their families. Both daughters and their husbands reside in Florida with his grandchildren.
MONTE REID SICHELMAN, M.D

Dr Sichelman, one of the area’s leading Board Certified Ophthalmologists is one of the only Lakeland eye surgeons to perform the latest, proven BLADELESS LASIK surgery. While many advances have been made in LASIK technology over the years, the most recent breakthrough with Bladeless LASIK is revolutionary. Bladeless LASIK now offers patients who were not previously candidates for LASIK the opportunity to see clearly, near or far, without glasses or contacts.
As a Corneal subspecialist, he provides complete care for Corneal diseases and transplants. Dr Sichelman received his medical degree and completed his residency at the University of South Florida. He graduated from the University of South Florida, College of Business, with a Masters of Business Administration. He graduated from Stanford University, with a Bachelor of Arts, Economics Degree. Dr. Sichelman is a member of the American Academy of Ophthalmology, and the Tampa Bay Ophthalmology Society.
Dr. Sichelman completed his fellowship training and is a corneal sub-specialist having performed numerous corneal transplants. He assists patients in improving their visual acuity using the blade-less laser system to provide refractive surgery.
He enjoys all sports, especially tennis and golf, as well as collecting baseball, basketball, football and hockey cards. He and his family reside in Lakeland.
AHMAD TARABISHY, M.D.

Dr. Tarabishy’s clinical expertise include uveitis and systemic inflammatory diseases, macular degeneration, diabetic retinopathy, and surgical treatments of various retinal diseases including complex diabetic and rhegmatogenous retinal detachments. He is passionate about retina surgery and employs the latest cutting-edge surgical techniques. He has published over 15 articles in major, peer-review scientific journals and is a co-author for 2 book chapters in major ophthalmology textbooks.
Ahmad Bakir Tarabishy grew up in Brooksville, FL and graduated with highest honors from Central High School in 1997. He completed a combined 6-year undergraduate and medical school program and received his medical degree from the University of Jordan Medical College. He then completed a residency in Internal Medicine at the Cleveland Clinic. After residency, Dr. Tarabishy worked as a Research Associate at Case Western Reserve University where his research helped describe important inflammatory pathways in ocular infections.
Dr. Tarabishy then worked at Massachusetts General Hospital, Harvard Medical School, as a clinical and research fellow in rheumatology before continuing on to complete his ophthalmology residency at the world-renowned Cleveland Clinic Cole Eye Institute, learning from some of the most prominent figures in ophthalmology and vitreoretinal surgery. After his ophthalmology residency, Dr. Tarabishy completed a 2 year fellowship in vitreo-retinal surgery at the Havener Eye Institute of The Ohio State University.
Dr. Tarabishy is a board certified ophthalmologist and a fellowship trained retina specialist. He is a member of the American Society of Retinal Specialists, the American Academy of Ophthalmology, the Tampa Bay Ophthalmology Society, and the American Medical Association. He is also board-certified by the American Board of Internal Medicine.

He is a peer reviewer for Ophthalmology and JAMA Ophthalmology and has been a contributor to Encyclopedia of the Eye.
STEPHEN M. BUTLER, M.D.
BOARD CERTIFIED surgeon
Stephen M. Butler, M.D., is a board-certified surgeon and a fellow of the American College of Surgeons. He has been affiliated with South Florida Baptist Hospital since 1992, is a past president of the hospital medical staff and has served on every major medical staff committee.
Dr. Butler practices general and vascular surgery with a special interest in laparoscopic surgery; hernia and aneurysm repairs; and colorectal, carotid, breast and thyroid surgery. In addition, he performs endoscopy, peripheral vascular surgery and sentinel lymph node evaluation for breast cancer. Dr. Butler is nationally recognized in hernia repairs and teaches physicians the latest techniques in hernia repair and endoscopic suturing for treatment of gastroesophageal reflux disease (GERD).
A graduate of the University of Missouri-Kansas City School of Medicine, Dr. Butler completed his residency training in surgery at St. Luke's Hospital in Kansas City. He received further training in his field through an endoscopy fellowship at Mt. Sinai Hospital in Cleveland, Ohio. Prior to moving to the Plant City area, Dr. Butler was in private practice in Missouri.
Dr. Butler supports numerous local charitable causes, including the Spring, the Rainbow Ball, South Florida Baptist Hospital Foundation and St. Leo's College.
ROBERT APPLEBAUM, M.D.

Dr. Applebaum specializes in cosmetic and reconstructive surgery of the eyelids and face in both adults and children. These surgeries include upper and lower eyelid blepharoplasty, eyebrow lifts, and repair after skin cancer removal among many others. Dr. Applebaum also specializes in cosmetic injectables, such as Botox, Juvederm, Voluma, Restylane, and Kybella. Dr. Applebaum uses these and other aesthetic techniques to provide comprehensive patient care.
Dr. Applebaum grew up in the Hyde Park neighborhood of Tampa, FL. He attended high school at Berkeley Preparatory School. He then spent his freshman year at Georgetown University in Washington, DC before transferring to the University of North Carolina at Chapel Hill, where he completed his bachelor’s degree in Public Policy with a specialization in Health Policy. He completed a combined MD and MBA program at the University of Florida in Gainesville, FL, graduating in the top 1/3 of his medical school class and receiving the award for highest business school GPA. After medical school, Dr. Applebaum completed a one-year Transitional internship at the Virginia Tech/Carilion School of Medicine. He spent three years at the University of South Florida, where he completed his Ophthalmology residency (in June 2015). He then completed an additional year of fellowship training in Oculofacial Plastic and Reconstructive Surgery at the J. Justin Older Eyelid Institute.
As a native Floridian, Dr. Applebaum enjoys year-round golf and tennis, which he plays frequently with his father. During the winter months, he prefers to don his skis and hit the slopes as often as possible (which is never often enough). Dr. Applebaum enjoys collecting and tasting wines from around the world (ask him about his “dissertation email” on Napa Valley). He also enjoys cooking, eating, and listening to live music. He is susceptible to flattery and enjoys being called a renaissance man by both peers and complete strangers.

MARLEN LEON, M.D.
BOARD CERTIFIED PEDIATRIC OPHTHALMOLOGIST
Eye Exams and Pediatric Eye Surgery including Strabismus Surgery
(Eye Muscle Surgery), Tear Duct Surgery, Pediatric Cataract Surgery and Chalazion Surgery.
Dr. Leon attended medical school in Florida. She is Board Certified in Ophthalmology and Florida Licensed. Dr. Leon successfully completed her pediatric ophthalmology and strabismus fellowship at Emory University in Atlanta, Georgia. She is prepared to care for our area children as well as to see patients with adult strabismus and other eye conditions.
Dr. Leon sees patients from birth to 18 years of age for all eye problems (failed vision screenings, blocked tear duct, eye misalignments…) and is fluent in Spanish. She performs Routine Eye Exams, Medical Eye Exams and Pediatric Eye Surgery including Strabismus Surgery (Eye Muscle Surgery), Tear Duct Surgery, Pediatric Cataract Surgery and Chalazion Surgery.

evan dunn, M.D.
BOARD CERTIFIED OPHTHALMOLOGIST
Dr. Evan Dunn graduated Magna Cum Laude from the University of Central Florida. He subsequently completed medical school at the University of Miami, where he was awarded the Tod Gassen International Fellowship. His Transitional year and Ophthalmology residency were completed at Indiana University, where Dr. Dunn was Chief Resident of the Transitional Year program. Dr. Dunn was instructed by leaders in the field of retina, uveitis, glaucoma, oculoplastics, pediatrics, and neuro-ophthalmology during his residency. While a resident, Dr. Dunn created a surgical skills program to augment resident training and participated in community service providing care to the uninsured. He was also the inspiration for the “Evan Dunn Ophthalmology Scholarship” created in 2016 by a thankful and generous veteran.
Dr. Dunn completed his training in vitreoretinal surgery & diseases at a combined fellowship between the University of Illinois at Chicago and University Retina. Here, he underwent training with internationally and nationally recognized leaders in the field of medical and surgical retina.
Dr. Dunn has presented at several institutions, regional and national meetings, and published articles. He has also participated in several clinical trials.
Dr. Dunn is dedicated to providing the highest quality medical and surgical care to patients in a friendly environment. His clinical interests include the treatment of diabetic retinopathy, macular degeneration, and retinal vein occlusions. For patients requiring treatment with intravitreal injections, Dr. Dunn employs a painless technique to maximize patient comfort. He has expertise in the surgical management of dropped lenses, macular holes, epiretinal membranes, rhegmatogenous retinal detachments, proliferative vitreoretinopathy, and tractional retinal detachments.
Teofil B. Kulyk, M.D.
BOARD CERTIFIED OPHTHALMOLOGIST
Dr Teofil B. Kulyk was a member of the group of physicians who brought Central Florida SurgiCenter to the Lakeland community. While providing care to his patients since 1980 at his Plant City Office, he continues to provide surgical services to his patients at the Center.
Dr Kulyk is a fellow of the American Academy of Ophthalmology and on staff at South Florida Baptist Hospital in Plant City. He also dedicates his time as a member of the Plant City Chamber of Commerce.
In his spare time, Dr Kulyk enjoys being a concert pianist and oftens shares his talent to help raise funds for local charities.
